What is asthma?1
Asthma is a long-term illness that makes the airways in your lungs narrow, swollen, or blocked. It is important to do everything you can to keep your asthma under control.
Uncontrolled asthma can:
- Make it hard for you to breathe.
- Keep you from playing sports or doing physical activities.
- Cause you to miss work or school.
You can control asthma if you:
- Know your triggers and how to avoid them.
- Take your daily medicine as directed by your provider.
- Use your rescue inhaler as directed by your provider.
What can the Asthma program do for me?
The Asthma program may improve your quality of life by providing education and services that help you:
- Understand what triggers your asthma, through a focused asthma assessment and care manager outreach.
- Learn the early warning signs of an asthma attack.
- Make an asthma action plan that empowers you and promotes self-management of the illness.
- Understand and coordinate your daily and rescue medications.
You have the right to choose not to participate and opt out of any program at any time. To opt out, please call 1-833-643-2262. You do not have to participate in the program to continue to receive services from the plan.
